Diabetes Education

Programs

Our medical clinic in Lincoln has an onsite Diabetes Self-management Education Program that is accredited by the American Association of Diabetes Educators.  All educational sessions are free of charge.  Our program offers a variety of services including:

  • Community Diabetes Education Classes

    These group classes are offered on a monthly basis free of charge to all members of the community. Classes address common topics of interest to individuals with diabetes and pre-diabetes including blood sugar monitoring, medication management, healthy eating, being active, risk reduction, problem solving and healthy coping. You do not have to be a patient at the clinic to attend. Registration is recommended.

  • Individual Diabetes Education Sessions

    Individual sessions are available for patients with diabetes and pre-diabetes who receive primary medical care through our clinic. Family members and those who support them in their disease management are also encouraged to attend. The Diabetes Educator will support the patient in goal setting, barrier identification, and evaluating progress. The Diabetes Educators work closely with the medical providers to ensure care is based on Indian Health Service and American Diabetes Association Standards of Care. Appointments are required.

Individual diabetes education sessions address:

  • Blood Sugar Monitoring

    Meter set up and use, blood glucose log review, and pattern identification.

  • Medication Management

    Review of medications and their effect on diabetes.

  • Healthy Eating

    Carbohydrate counting, meal planning, healthy eating on a limited budget.

  • Being Active

    Review the role of physical activity in managing and preventing diabetes and assist patients in developing a plan for activity that fits their ability and lifestyle.

  • Risk Reduction

    Identification and treatment of high and low blood sugar, foot care, smoking cessation, immunizations, routine medical appointments and recommended lab studies.

  • Problem Solving

    Support with identifying difficulties and finding solutions to improve overall diabetes management.

  • Healthy Coping

    Stress management, importance of supportive relationships, and the role of mental health, culture, and spirituality in managing diabetes.

Additional diabetes related services offered by our clinic include:

  • Podiatry care offered monthly
  • Immunizations
  • Medication assistance to those who qualify
  • Diabetes Case Management to those who qualify

  • Those who are newly diagnosed with diabetes or prediabetes
  • Those who are starting a new diabetes medication or insulin
  • Those with high blood sugar/elevated HgbA1c levels
  • Those with diabetes who are planning to become pregnant
  • Those with diabetes or pre-diabetes who are new to the clinic
